The name of the third author, Paulo Lotufo, has been corrected to Paulo A. Lotufo. The name of the fourth author, Isabela Bensenor, has been corrected to Isabela M. Bensenor. The name of the seventh author, Peter Toth, has now been corrected to Peter P. Toth. The authors regret the errors.
